Housing Stabilization Provider of Accessible Housing
Our organization is dedicated to providing housing for those experiencing homelessness. We are committed to offering opportunities to every individual or family in need of shelter ensuring that no one is left without safe and stable housing. We are a licensed provider through the state of Minnesota that operates under the (HSS) housing stabilization service regulations.
WHAT IS HSS?
Housing Stabilization Services (HSS) is a new Medical Assistance (MA) benefit designed to help people with disabilities or seniors find and keep housing. Housing Stabilization Services is a home and community-based service. We take pride in offering great service and we are always looking for way to improve and grow and now we’re expanding right here in Minnesota.

Housing Support for a Brighter Future
Our Goals for Housing Stabilization Services are to Support an individual's transition into housing, increase long-term stability in the community, and avoid future periods of homelessness or institutionalization. We find and work on various barriers that can and do make it hard to find housing, interact with landlords and neighbors, and understand the rules of a lease. We also help individuals with budgeting their income, setting up auto payments, and more. Finding and keeping stable, safe, healthy, and affordable housing is our priority for our fellow residents of Minnesota. We offer Transition and Sustaining Housing Services. This is all we do, and we do it well.
Please Note: We can assist all individuals who have Medical Assistance. If you do not have a case manager or targeted case manager and are a non-waiver individual (which is not cadi waivered), we can assist you through the process of being approved for this service.
Eligibility Requirements for Our Housing Program
Our services are part of a state-funded medical program designed to assist individuals in need of low-income housing. To qualify, you must meet certain criteria, including having Medical Assistance (MA), a qualifying disability, and an approved Person-Centered Service Plan (PSN) or Community Support Services Plan (CSSP).
