Qualification Requirements:

At our company, we are dedicated to helping individuals with disabilities who are 18+ and have medical assistance, facing housing instability, or experiencing homelessness.

Additionally, we work with adults who have waivers such as CADI, Elderly Waiver, or other nonstandard waiver services.

We also help those without waivers to be approved as non-waivered. By completing a Professional Statement of Need and a Housing-Focused Person-Centered Plan, we can assist you in navigating the process and connecting you with the necessary resources.

Let us help you take the next steps towards a more independent and fulfilling lifestyle.
