At our agency we understand the challenges that can come with finding and maintaining stable housing. Whether you are in need of funding for new housing or past due rent. If you are currently homeless and in need of shelter we are here to help. Our team can assist you in creating a long-term stability action plan to help you get back on your feet.Â
We specialize in helping individuals search for and apply for affordable housing in their chosen location. Understanding the rental process and your new lease before you sign is crucial. Our experts can guide you through this process. Additionally, we can work with you to create a budget plan and find funding for deposits and moving costs.
For those in need of transitional services or who require updated HFPCPs to reflect moving expenses. We are here to assist. Our team can help navigate housing issues with management and other residents. Ensuring that you feel supported throughout the process. If applicable, we can assist in understanding and applying for disability services and other financial assistance programs.
After getting you housed, we will continue to work with you on a stabilization plan. To ensure that you do not lose access to your new housing.
Housing Stabilization Services for Individuals
Housing stabilization services is a new service that is funded by Medical Assistance health insurance through the state of Minnesota. This service helps people find and sustain housing.
This service is available to individuals aged 18 and older who have case management, are Cadi Waivered or non-waivered, and are facing challenges related to mental or physical disabilities, substance abuse disorder, or are 65 years or older and on medical assistance. We encourage those who meet these criteria to apply.
Clients may be referred by a case manager, care coordinator, or can refer themselves. Two different forms are required for each type of referral: one for self-referrals and one for referrals made by a case manager or care coordinator.
